Reasoning, not recall
A trainee who knows every cause of chest pain can still be poor at chest pain, because the clinical skill is not retrieval of a list but the ordering of that list against a particular patient in front of you. That ordering is learnt from volume — from having seen enough presentations that the shape of one becomes familiar.

That organisation matters: patients present with complaints, and a book organised by diagnosis can only be used by someone who already has the answer.
