A sequence, not a collection
Most ECG books are either too elementary to survive past the second year of training or assume a fluency the reader does not yet have. The 59 chapters here are ordered so that nothing is used before it has been established: rate, rhythm, axis and intervals first, then chamber abnormality, then ischaemia and infarction territory by territory, then conduction disease, then the arrhythmias, then the patterns that only make sense once all of it is in place — Brugada, ARVC, the long and short QT syndromes, Wellens, de Winter, the ECG of the post-arrest patient.
The result is a book a house officer can start and a fellow can finish, which is unusual and is the reason the Academy uses it across two training stages rather than one.
Where it sits in the pathway
Chapters 1 to 24 are foundation reading, recommended to Academy members in internal medicine, emergency medicine and critical care as well as to cardiology trainees. Chapters 25 onward are advanced reading for the cardiology track. Members frequently report returning to the middle chapters years later, which is the honest test of an ECG text.
The interpretive discipline it teaches
Every chapter enforces the same reading order, because the commonest interpretive error in practice is not ignorance but sequence — the striking abnormality is seen first and the systematic read is abandoned. A reader who has worked through this book has the sequence drilled to the point where the striking finding is noticed in its proper place.
Clinical correlation is stated explicitly: what the tracing means for the next decision, when it changes management within minutes, and when it does not change management at all. The infarction chapters give occlusion patterns their proper weight, including the territories that are routinely under-called.
