Heart failure is the area of cardiology where a textbook from ten years ago is actively dangerous. Four-pillar therapy, the SGLT2 inhibitor evidence extending across the ejection fraction range, the collapse of the old HFpEF nihilism, the redefinition of device candidacy — a physician working from older material is not merely behind, they are treating a different disease.
This collection is the Academy’s answer to that. It is the designated reading for the heart failure track and the one members are told to prioritise if they can only take one collection.
Structure
Diagnosis and phenotyping first, including the natriuretic peptide thresholds that change with obesity and atrial fibrillation and the echo criteria that separate HFpEF from its mimics. Then guideline-directed medical therapy as a sequencing problem rather than a list — which agent first, how fast to titrate, what to do when blood pressure or renal function objects, and which of the four pillars to sacrifice when all four cannot be tolerated.
Acute decompensation is handled separately: congestion phenotypes, diuretic strategy including sequential nephron blockade, the point at which a rising creatinine means the treatment is working, and the recognition of the patient who is not merely decompensated but in low output.
Devices and advanced therapy
ICD and CRT candidacy against the DANISH-era evidence rather than the older assumptions. Cardiac contractility modulation. The referral decision for advanced therapy — the single judgement Academy members most often ask about, given how much outcome depends on referring before rather than after end-organ damage. Mechanical support and transplant assessment are covered to the level a general cardiologist needs in order to refer competently.
