This is the submission form for the Academy’s recognition programme. Before completing it, read Apply for Academy Awards, which sets out the three categories and what the panel assesses.
What you will need before you start
Have the following to hand: your full name exactly as it appears on your primary medical qualification; your degrees and post-nominals; your current medical registration or licence number and the issuing council; your present post, department and institution; the city and country you practise in; and a working email address that you check.
You will also need your supporting statement of no more than 800 words, and the names, positions and contact details of two independent referees. A referee should be someone who has seen the work rather than someone senior who has heard of it.
How your details are used
Registration details are verified against the issuing council. Nothing you submit is published without your written consent, and referees are contacted only for shortlisted candidates. If your application is unsuccessful your file is retained for one cycle in case you wish to resubmit, then deleted. Full detail is in the privacy policy.
After you submit
You will receive an acknowledgement by email. Panel review takes place in cycles rather than continuously, so a decision may take several weeks. If you have not heard within a reasonable period, write to us through contact rather than resubmitting, which creates a duplicate file.
Academy members can track their submissions from their member account. If you are not yet a member, membership carries nomination rights at Platinum level and a verified directory listing at every tier.
