Paediatric cardiac surgery worked. The consequence is a large and growing adult population living with repaired congenital lesions, who present to general cardiologists with problems those cardiologists were never taught — because when they trained, these patients did not survive to become adults.

They now outnumber the paediatric congenital population, and they are looked after considerably worse.

Because the commonest error is not mismanagement but non-recognition — a repaired tetralogy patient presenting with breathlessness in their thirties, assessed as a general cardiology problem, with the pulmonary regurgitation that explains everything never considered. The common repaired lesions and what each fails from: tetralogy and pulmonary regurgitation, transposition after arterial or atrial switch, Fontan physiology and its slow attrition, coarctation and late hypertension, unrepaired shunts and Eisenmenger physiology. Then the cross-cutting problems — arrhythmia, heart failure in an abnormal circulation, endocarditis risk, pregnancy, non-cardiac surgery, and contraception.
