“Cut down on sugar” is not a plan
It is the single most-issued piece of dietary advice in medicine and one of the least followed, because it names an outcome and supplies no method. Patients agree, reduce for a fortnight, and revert — usually concluding the failure was theirs.

Where the sugar actually is
The practical core. Most patients reduce visible sugar and leave intact the considerably larger quantity in sauces, breads, drinks, condiments and packaged foods. The programme is specific about label reading and about the products patients consistently misclassify as healthy.
Withdrawal and relapse
Both are covered without overstating the physiology. Patients who expect the first week to be unpleasant tolerate it; patients who are surprised by it stop. Setting that expectation is most of the adherence benefit.
